Common Trunk Reshaping Procedures
Dr. Gouverne considers your specific needs and concerns when considering the best surgical approach. He most commonly performs the procedures below (listed in order from most to least common):
- Tummy tuck with Agitation-Infusion Lipo-Sculpture (A.I. Lipo-Sculpture): Best for patients needing fat reduction, waist shaping, endo-fascial repair, and removal of excess abdominal skin.
- A.I. Lipo-sculpture: Best for patients with localized fat pockets but without muscle weakness or skin redundancy.
- Tummy tuck with A.I. Lipo-Sculpture and Brazilian butt lift (BBL): Best for patients needing fat reduction, waist shaping, endo-fascial repair, removal of excess tummy skin, and buttocks augmentation

What is a no-drain tummy tuck?
As the name suggests, a no-drain tummy tuck simply means that there is no drain needed during the healing process. In a no-drain tummy tuck, Dr. Gouverne tightly bonds the layers of tissue together, so there is no space for serous fluid (a natural consequence of healing) to accumulate.
A no-drain tummy tuck has 2 major advantages:
- It's easier. No postoperative instructions on taking proper care of drains and no invasive postoperative visits for drain removal.
- Recovery is quicker. Getting the drainage process out of the way shortens recovery time and requires fewer postoperative visits.
Drains are necessary when the surgeon performing a tummy tuck leaves a space between the tissue layers. Excess fluid accumulates in this area during the first week to 10 days after surgery (sometimes longer), and a drain removes that fluid as the area heals.
Although abdominoplasty can be customized based on the patient's preferences and areas of concern, most procedures require an incision that extends from hip to hip just above the pubic area. This incision allows for optimal removal of excess skin and fat, along with muscle repair. With proper postoperative care, the scar will fade significantly and be well-hidden by clothing, including swimwear.
Once the incision is made, Dr. Gouverne repairs separated abdominal muscles with internal sutures. The skin is pulled tight, and the excess is removed. Some navel reconstruction is often part of the abdominoplasty procedure as well.
Customized procedures may include innovative techniques developed more recently that reshape the trunk by addressing additional areas beyond the tummy. These techniques include:
- Agitation-Infusion Lipo-Sculpture (A.I. Lipo-Sculpture): Reduces unwanted fat and uses fat grafting and contouring to provide smooth results
- Endo-Fascial Repair (EFR): Stitching that repairs stretched bellies coupled with a continuous anesthetic infusion via an external pain pump
- Brazilian Butt Lift (BBL): A fat grafting procedure that augments the buttocks and contours the waistline
- Floating Umbilicoplasty (FUP): A no-scar technique to reposition the belly button
Dr. Gouverne performs abdominoplasty as an outpatient procedure, and patients are able to return home after a brief period of rest and observation.
Although plenty of rest is important for proper healing following surgery, it's imperative that abdominoplasty patients resume light activity, such as walking, as soon as possible. Mobility reduces the risk of blood clots and other complications and promotes healing. Dr. Gouverne advises patients to walk around briefly at least 4 times within the first 24 hours after surgery. Patients are also provided with a compression hose, which further reduce the risk of blood clots. Pain and discomfort following abdominoplasty are usually manageable with prescribed medication. Narcotics, including Demerol and Lortab, are often prescribed in tandem with anti-nausea medications. It's very important that you follow Dr. Gouverne's instructions carefully during your use of these drugs. Dr. Gouverne also offers a pain pump as an alternative to pills. He helps you decide which pain management option is best for you.
Experiences vary after abdominoplasty. Most patients are able to return to work after 2 weeks, although some may choose to spend more time at home depending on the specifics of their surgeries. Certain activities, such as those that include bending or heavy lifting, should be avoided.
